Position : Social Worker/Case Manager Supervisor
Due to a new requirement from the Centers for Medicare & Medicaid Services (CMS) the following mandate is effective January 27, 2022: As a condition of employment, employees providing health care services and/or support services in covered facilities must show proof that they have received the dose of a single J&J dose or the 1st dose of Moderna/Pfizer two-dose vaccine effective date January 27, 2022, and proof of the single J&J dose or the 2nd dose of Moderna/Pfizer two-dose vaccine by February. 28, 2022. Employees may not provide any care, treatment, or other services until proof is provided. Based on CMS guidance, the date for compliance may extend into March due to the health care facilities high vaccination rate of staff. If you accept a position with the facility, your Human Resources representative can provide more guidance. The only exception to these requirements is for those employees granted medical or religious exemptions from the COVID-19 vaccine as recommended by the Centers for Disease Control and Prevention, which may require weekly COVID-19 testing. A class incumbent is responsible for supervising daily activities of staff by coordinating case management and/or employment and training functions and assisting administrators in program assessments, planning and implementation.
Supervises a staff of 6-10 professional, paraprofessional and clerical employees.
Essential functions are fundamental, core functions common to all positions in the class series and are not intended to be an exhaustive list of all job duties for any one position in the class. Plans, coordinates, assigns, reviews and assesses unit workflow. Trains, hires, evaluates and supervises staff.
Participates in program planning, implementation and coordination to insure that program goals and current operations are in compliance with department and Federal/State rules, regulations, policies and procedures.
Gathers and assimilates statistics and information to insure program compliance.
Recommends and implements corrective action as necessary.
Coordinates with social service staff, agencies, providers and resources to provide comprehensive services for care and treatment of clients in a timely, efficient, and cost effective manner.
Represents the agency on task forces, committees and with the general public.
Provides information to clients, general public, community representative, and other agencies in order to explain programs and policies.
May plan and develop programs which includes writing grant proposals, completing all state clearing house documents, and implementing new grants when awarded.
Analyzes budget needs, develops recommendations regarding staffing, supply and equipment and may monitor current budgetary expenditures.
Prepares statistical and program evaluation reports.
JOB REQUIREMENTS for Social Worker/Case Manager Supervisor Applicants must have education, training and/or experience demonstrating competence in each of the following areas:
Three years experience in case management which includes assessing, planning, developing, implementing, monitoring, and evaluating options and services to meet an individuals human service needs.
Three years experience in making recommendations as part of a client’s service plan such as clinical treatment, counseling, or determining eligibility for health or human services/benefits.
Six months experience in interpreting laws, rules, regulations, standards, policies, and procedures.
Six months experience in using an automated information system to enter, update, modify, delete, retrieve/inquire and report on data.
Six months experience in narrative report writing.
Knowledge of staff supervision acquired through course work, academic training, training provided through an employer, or performing as a lead worker overseeing the work of others; OR supervising staff which may include planning, assigning, reviewing, and evaluating the work of others; OR supervising through subordinate supervisors a group of professional, technical, and clerical employees.
