Education and Experience:
Bachelor’s Degree in Behavioral Health, Psychology, Health Behavior Science, or related field.
Experience working with adults with severe and persistent mental illness and/or substance use disorders.
CPR and First Aid Certification.
Valid driver’s license and permission for a review of driving record to meet insurance requirements.
Responsibilities:
Professionally participate in team conferencing to create and implement care plans for clients.
Maintain a therapeutic alliance with clients.
Refer and link clients to all needed services provided by an outside agency.
Coordinating independent living assistants, medical and vocational services.
Work with community agencies to develop needed resources including housing, employment options, and income assistance.
Provide support and consultation with the client’s family.
Provide in-home services including ADL training, medication assistance and training, social support, and home management assistance; providing supportive counseling.
Document and keep other staff, clinical coordinators, and Program directors informed of all changes in the consumer’s mental or social status.
Carries primary or secondary cell phone as assigned and responds to after-hour and emergency calls in accordance with program directives.
Provides education and works as an advocate for persons served in the community.
